CHE Trinity Health, Sharecare Launch Engagement Platform

Healthcare delivery system CHE Trinity Health and health and wellness platform vendor Sharecare are partnering to provide consumers and patients with a personalized, localized engagement platform across the 20 states that CHE serves.


Healthcare delivery system CHE Trinity Health and health and wellness platform vendor Sharecare are partnering to provide consumers and patients with a personalized, localized engagement platform across the 20 states that CHE serves.

"The partnership between CHE Trinity Health and Sharecare will support individual and community health outside of the clinical environment in ways that are convenient for consumers," the organizations said. "In addition to engaging consumers in their wellness with informative and compelling digital content, CHE Trinity Health’s regional health systems will have new avenues for patient acquisition and online physician profiles."

Features of the platform include:

*RealAge Test health risk assessment tool;

*Individualized programs to promote general wellness in areas such as weight loss, smoking cessation, stress management and diabetes prevention;

*Digital health integrations, including the ability to connect health trackers and sensors, and Sharecare’s health consultation app AskMD;

*Tools to locate a CHE Trinity Health physician, schedule an appointment, and communicate with the physician; and

*Content to help patients prepare for surgery, therapy or other clinical services, and aid their recovery afterward.

CHE Trinity Health operates in 20 states with 86 hospitals, 109 continuing care facilities and home health and hospice programs that provide nearly 2.8 million visits annually. The organization was formed in May 2013 by the merger of Catholic Health East and Trinity Health.

Sharecare was created in 2010 by Jeff Arnold and Mehmet Oz, M.D. The company says nearly 40 million people have shared 5 billion data points with the company since its inception.
