MEMI Tech LLC in Louisville, Ky., has introduced a credit card-sized portable personal health record that plugs into any USB port on a computer using the Windows operating system.

The 911 Medical ID card includes a push-down USB tab. When inserted into a computer, a template opens to enable the user to enter family history, medical conditions and tests, allergies, prescriptions, living will, physician and emergency contacts, and other health information. The card, with two gigabytes of data, can hold the PHRs of 10 people. Information on the card is automatically uploaded to a secure Web site, giving a Web-based option for maintaining the PHR. A wallet or purse sticker identifies the user as having the PHR available.
