6 Steps to Population Health Management
Population health management involves a six-step lifecycle incorporating a range of skills and technologies that healthcare organizations must successfully adopt and support, according to a new study from Chilmark Research.
While there is a natural progression through the PHM lifecycle, the ability to accomplish the various tasks that are outlined is fraught with challenges, from lack of sufficient IT infrastructure to poor process workflows, cultural alignment and the strain on financial resources to make it all happen. Compounding these challenges is simply the lack of best practices in the industry that would help define how an organization navigates its way along the journey to a successful PHM program.
