Wellframe, a cloud-based mobile care plan platform for the chronically ill, has been named the grand prize winner of the Merck/Heritage Provider Network Innovation Challenge.

The challenge invited innovators and entrepreneurs to submit their concepts for products and services to address the pain points associated with care plan adherence for people affected by diabetes and/or cardiovascular disease.

In a blog post describing the winning technology, the organizers said, "In addition to potentially helping care providers scale services while maintaining patient-centricity, Wellframe’s mobile application translates the care plan into a manageable, dynamic, and personalized daily to-do list. After using the application, patients have reported feeling in more control of their health."

Wellframe says its technology includes a proprietary approach to representing clinical protocols as logic structures to direct computer systems at scale and across clinical verticals; a patient interface including a dynamically generated, personalized daily to-do list, a secure two-way communication channel, multimedia medical information, and a private social network; a HIPAA-compliant, cloud-based data collection system; and a cloud-based care management EMR that has been successfully integrated into clinical workflows.
