UNC Health Care in Chapel Hill, N.C., and Irvine, Calif.-based healthcare delivery framework vendor Alignment Healthcare have formed a strategic and operational partnership for population health management and will offer a Medicare Advantage HMO product for residents of Wake County, N.C.

To care for the seniors who enroll in the new plan, UNC Health Care--including UNC Physicians Network, UNC Faculty Physicians and Rex-employed specialists, Rex Hospital, and UNC Hospitals--will serve as the foundation of the provider network, while "select, high-quality community providers" will also be in the network to ensure seniors have access to a full continuum of care close to home.

Alignment Healthcare offers a fully integrated care coordination platform for population health management which it says includes "an advanced clinical model, IT and enablement systems, risk experience, and capital."  Earlier this year, Alignment secured a $125 million commitment from global growth equity firm General Atlantic.

The parties say the new plan will begin offering coverage in January 2015.
