An unauthorized individual or company accessed and copied all or parts of the electronic health records system at Sight and Sun Eyeworks Gulf Breeze in Gulf Breeze, Fla.

“Sight and Sun Eyeworks Gulf Breeze believes the records were accessed only to offer other medical services and not to commit identity theft,” according to a notice from parent company Gulf Breeze Family Eyecare Inc. “However, patients should closely monitor their credit card, bank and other financial statements for signs of fraud and identity theft.”

The notice did not indicate the scope of the breach, but the Pensacola News Journal reports about 9,000 patients were affected. Clinical and demographic information, along with Social Security numbers, were among the compromised data. A company official did not respond to a phone call asking for more information.
