Wright-Patterson Medical Center in Dayton, Ohio, soon will have two listings on the federal government’s list of major breaches of protected health information.

The organization has notified 3,800 patients after a notebook was left overnight in a conference room and recovered the next morning behind a chair, apparently after being dropped, according to the Dayton Daily News. Names and Social Security numbers on sign-in sheets were among the information on the notebook.

The medical center did not respond to a Health Data Management request for more information. Wright-Patterson in July 2010 had a breach resulting from improper disposal of paper records that affected 2,123 patients.
