Lewin Group Gets CMS Care Transitions Contract

The Centers for Medicare and Medicaid Services recently launched the $500 million Community-based Care Transitions Program to help hospitals improve their care transition processes and reduce readmissions.


The Centers for Medicare and Medicaid Services recently launched the $500 million Community-based Care Transitions Program to help hospitals improve their care transition processes and reduce readmissions.

Now, CMS has contacted with consultancy The Lewin Group to provide technical assistance and guidance to about 500 community based organizations and hospitals expected to participate in the program funded under the Affordable Care Act.

The five-year contract to The Lewin Group is valued at $2.3 million for the first year. The consultancy will gather and disseminate best practices from site visits and facilitate peer-to-peer information sharing through online collaboration and national meetings. Subcontractors include the Colorado Foundation for Medical Care, University of Colorado, Seamon Corp., 371 Productions and ON24. The Lewin Group is a unit of OptumInsight, formerly Ingenix.
