Illinois rural organization picks Cerner analytics platform for pop health

Illinois Rural Community Care Organization in Princeton, Ill., has selected the HealtheIntent data analytics platform of Cerner to support population health management initiatives that include an accountable care organization to coordinate care for Medicare patients throughout the state. “We think Cerner’s population health solutions will help the doctors and nurses at our member hospitals feel confident in their decision making,” says Pat Schou, executive director at the Illinois Critical Access Hospital Network. “Comprehensive and comparative reports which include information on the care individual patients receive, and the overall health of our patients, support the ACO in targeting its strategies to improve the delivery and cost of care by providers.”

Lawrence General Hospital in Massachusetts will implement the Expanse next-generation web electronic health record platform of Meditech, increasing the vendor’s footprint in the state where it already has a 57 percent market share. Lawrence General is a 189-bed facility serving 94,000 residents in Lawrence as well as 21 surrounding communities. Acute and ambulatory care implementation will start in July, with plans to be live in the spring of 2019. A key factor for selecting Meditech was the desire to have a fully integrated system, particularly in the emergency and laboratory departments as the hospital seeks to achieve Stage 7 of the HIMSS Analytics scale of EHR excellence. Santa Rosa Consulting is working with the hospital.

Massachusetts General Hospital and Brigham and Women’s Hospital in Boston will implement the IntelliSite Pathology software of Royal Philips. The goal is to better enable research and support clinical diagnoses. The organizations will jointly deploy digital pathology infrastructures to enable research, education and collaboration, and over time to shape and inform the deployment of digital pathology across the nation via best practices and protocols.

Partners Healthcare in Boston is expanding use of the ProviderMatch suite of patient access software from Kyruus. The delivery system has been using the vendor’s provider data management platform and provider matching software to help patients find the right providers on the Partners web site. Now, Partners will implement an enterprisewide patient access platform with ProviderMatch helping patients and call center employees to identify the right providers for patients’ clinical needs, logistical requirements and personal preferences.

Radford Green, a senior rehabilitation facility in Lincolnshire, Ill., is live on the enTouch communications platform of PreparedHealth to improve care coordination. Primary care physicians, nurses, physical therapists and home health workers—all assisting the same patient—can see the work being done and receive updates in real-time. Software from the vendor also gives information on what isn’t working for the patient with the goal of reducing readmission rates. PreparedHealth pilot tested in the Chicago area and now will expand across the state.
