Houston Methodist brings real-time patient notification to docs

Execs believe application can help coordinate care across the sprawling delivery system.

The accountable care organization of Houston Methodist, which includes Texas Medical Center and six community hospitals, will implement technology to automate notifications to physicians of patient admissions or discharges at any of the hospitals, as well as affiliated healthcare facilities.

The organization has contracted for the notification software of PatientPing. The ACO, called Houston Methodist Coordinated Care, serves Medicare fee-for-service patients assigned to the delivery system’s primary care network.

The goal is to offer timely interventions to improve the quality of care coordination by preventing duplication of services, reducing avoidable readmissions and making care transitions safer.

Real-time notifications include a patient’s visit history, enabling providers to see treatments that were delivered by other providers within an organization and at other treatment facilities.

“This will allow us to coordinate patient-centric care across the continuum of healthcare facilities from hospitals, emergency care centers, urgent care centers, skilled nursing facilities and post-acute care facilities to enhance our patients’ care,” says Julia Andrieni, MD, president and CEO at Houston Methodist Coordinated Care.
