Hospitalists Battle Readmissions

Since October 2010, more than 4,400 physicians have participated to some degree in an online program, also available via mobile devices, to prevent hospital readmissions for high-risk patients.


Since October 2010, more than 4,400 physicians have participated to some degree in an online program, also available via mobile devices, to prevent hospital readmissions for high-risk patients.

The Society of Hospital Medicine, which represents more than 31,000 hospitalists, created the program with QuantiaMD, an online physician community. The organizations last fall conducted a Web-based survey of more than 1,000 hospitalists that found 71 percent did not believe their organizations' processes for preventing readmissions were effective.

Since then, hundreds have viewed Web-based presentations, 2,700 have participated in online discussions and almost 1,000 have downloaded a toolset called Project BOOST from the Society of Hospital Medicine. The tools include a risk assessment screener, transition record, return on investment assessment and patient education materials.
