Traditional Home Health and Hospice in Dunmore, Penn., will implement Web-based monitoring systems in the homes of patients with congestive heart failure.

The company will use the Electronic House Call system of ExpressMD Solutions LLC, formerly EncounterCare Solutions, Berkeley Heights, N.J. The system enables patients to collect vital signs and transmit them via a secure Internet connection to clinicians. The service sends alerts to clinicians when a patient's data is outside a pre-set range.

The clinicians also can use the service to communicate with their patients, such as to schedule care plan reminders or disease management education.
