The system enables patients to collect vital signs and transmit them via a secure Internet connection to clinicians. The service sends alerts to clinicians when a patient's data is outside a pre-set range.
The clinicians also can use the service to communicate with their patients, such as to schedule care plan reminders or disease management education.
