The Agency for Healthcare Research and Quality is seeking input from patient safety organizations and software developers on technical specifications to support standard electronic reporting of patient safety incidents.

"The Patient Safety Act and Patient Safety Rule require PSOs, to the extent practical and appropriate, to collect patient safety work product from providers in a standardized manner in order to permit valid comparisons of similar cases among similar providers," according to an AHRQ notice published March 25 in the Federal Register.

AHRQ has developed a set of common definitions and reporting formats, called Common Formats, to aid in collecting and submitting standardized information. The agency is inviting PSOs and developers to a meeting on May 11 in Rockville, Md., to discuss technical specifications that give direction to developers planning to implement the Common Formats electronically. Agency officials also will provide information on specifications for updates to the formats, as well as the new Skilled Nursing Facility Common Formats.
