Six-hospital Memorial Healthcare System in Hollywood, Fla., is notifying about 9,500 patients who may have had their names, dates of birth and Social Security numbers improperly accessed.

Subsequent investigation has found that two employees, since fired, during 2011 and early 2012 accessed the information with the intent of filing false tax returns. Memorial Healthcare learned of the breach on January 27 and began notifying patients on April 12.

The delivery system delayed notification at the request of law enforcement officials, according to a notice published on its Web site. It is offering affected patients one year of free credit monitoring services, according to the Miami Herald.
