App Manages Post-Discharge to Cut Readmissions

Phytel Inc., a Dallas-based vendor of care and disease management software, has introduced a new application to help hospitals reduce readmission rates.

The Hospital Readmission Management product is the first in a new service line to automate post-discharge care processes. The application sends a telephone survey to patients within 72 hours of discharge that includes questions about their satisfaction of care and whether they understand the discharge instructions. Patients who would like more information then can be transferred to a hospital nurse help line or call center.

The software flags patients who do not respond to the survey so hospital care management staff can follow-up and assess how well the patients are doing. The Hospital Readmission Management software is on the same platform as Phytel's ambulatory care management software to enable delivery systems to integrate responses to post-discharge surveys with patients' primary care physicians.
