The Agency for Healthcare Research and Quality has released for public comment version 1.2 of its Common Formats set of definitions and reporting formats for hospital reporting of patient safety events to Patient Safety Organizations.

New features in Common Formats—Hospital Version 1.2, include new content for two event-specific formats: Venous Thromboembolism and Device or Medical/Surgical Supply including Health Information Technology Device.
AHRQ also will develop Common Formats for the ambulatory setting. More information is available in a notice published April 13 in the Federal Register.
