As value-based care takes hold, healthcare organizations are looking to better manage populations of patients to ensure care is timely, effective and as inexpensive as possible. Information systems are crucial in both collecting and analyzing data, and developing actionable results that can impact care.
KLAS Research, which studies the use of IT in healthcare organizations, sees six areas of functionality that are essential in population health management solutions. These include data aggregation, data analysis, care management, administration and financial reporting, patient engagement tools and clinician engagement.
In a recent report on population health management, KLAS Research identified vendors that are among those most depended upon by providers to help them direct care of various patient populations. This list summarizes KLAS’ study of the various leading vendors of systems with population health capabilities.
Corporate headquarters: Chicago, Ill.
Allscripts offers population health management solutions that work in tandem with its electronic health records system. Its platform is a combination of both acquired and native solutions. Some of the tools under the CareInMotion solution are dbMotion, Care Director, Care Management, Care Team Management, Referral Management, CarePort, Chronic Care Management, FollowMyHealth and Population Health Anayltics.
2. Arcadia Healthcare Solutions
Corporate headquarters: Burlington, Mass.
Arcadia combines population health management tools with consulting services. The tools were originally built to be used by their consulting clients, though non-clients now are using its applications—customers now include clinics, large national acute care organizations and health plans.
Corporate headquarters: Watertown, Mass.
The company offers a vendor-agnostic PHM suite that includes tools for claims data and business intelligence functions. The company also engages in risk-based profit sharing with some of their PHM customers.
Corporate headquarters: Bellevue, Wash.
Caradigm was formed as a joint venture by Microsoft and GE Healthcare; it has built out broad functionality across various PHM verticals, although with less focus on verticals like patient and clinician engagement. Most of their PHM applications reside on the Caradigm Intelligence Platform. Its customer base consists of mostly large organizations, each with many hospitals and clinics.
Corporate headquarters: Kansas City, Mo.
Cerner’s HealtheIntent platform for PHM is EMR-agnostic, aggregating data on the front end and then serves it up to several different applications, ranging from registry and analytics to care management solutions.
6. Conifer Health Solutions
Corporate headquarters: Frisco, Texas
Conifer Health Solutions is a subsidiary of Tenet Healthcare. Its customers consist of hospitals, health plans and employer groups. It has expanded its capabilities and now has comprehensive offerings that include both software and professional services. Its typical customer is a large acute-care organization.
Corporate headquarters: Westborough, Mass.
The company’s Care Coordination Medical Record has several different modules for data exchange, analytics, patient engagement and care management, working in a coordinated fashion with eClinicalWorks’ EMR and practice management systems. The application is not sold outside of its EMR customer base.
8. Enli Health Intelligence
Corporate headquarters: Beaverton, Ore.
Originally developed by Providence Health & Services, its solution was developed to work with GE Healthcare technology and is often deployed by users of the GE Healthcare Centricity EMR, meaning that many of its customers are ambulatory organizations. Even so, its solution is vendor agnostic and works with all kinds of EMRs. Enli is beginning to scale up into larger organizations.
Corporate headquarters: Verona, Wis.
The Epic population health management functionality falls under the umbrella name of Healthy Planet and is part of the Epic EMR. As a result, the company’s customers don’t have to deploy an additional module to get access to Epic’s population health functionality.
10. Forward Health Group
Corporate headquarters: Madison, Wis.
The company has many ambulatory practice customers, but clients also include medical centers and health systems. Typical population health customers tend to be involved in accountable care organizations, quality initiatives and pay-for-performance programs.
11. GSI Health
Corporate headquarters: Philadelphia
GSI specializes in bringing information across the community into population health initiatives. Another strength of GSI Health is helping customers identify the social determinants of health that are relevant to their patient populations. Many of GSI Health’s customers report information to government entities in an effort to help address payment reform with Medicare and Medicaid. GSI Health’s customer base includes hospitals, health systems, health plans/managed care organizations, ACOs, DSRIP programs and others.
12. Health Catalyst
Corporate headquarters: Salt Lake City
Health Catalyst’s analytics and data visualization capabilities have been used by healthcare organizations for years, and many of the customers for these products are beginning to use its population health management applications to further leverage their data. Even those that don’t are using its enterprise data warehouse for activities related to population health management.
Corporate headquarters: Piscataway, N.J.
A recent entrant to the PHM field, HealthEC has been processing claims data since the 1990s. Its customer base is mainly composed of midsize acute care organizations—many of its clients are independent, physician-led organizations, and they tend to need to bring together data from a wide variety of sources.
14. i2i Population Health
Corporate headquarters: Franklin, Tenn.
The company’s core offering is a registry, care management and analytics tool used to identify high-risk patients who need care management. It also has a data analysis tool. Its offerings are primarily focused on ambulatory providers, and their customers are typically midsize to large practices.
15. IBM Watson Health
Corporate headquarters: Armonk, N.Y.
IBM Watson Health’s population health management suite combines several acquired and internally developed solutions including Explorys, Phytel, Watson Care Manager and Truven. Many of IBM’s customers came as a result of these acquisitions. Explorys is focused on data aggregation and analysis. Phytel Atmosphere is known for care management and patient engagement functionality.
16. Lightbeam Health Solutions
Corporate headquarters: Irving, Texas
The company offers a cloud-based, vendor-agnostic PHM solution that includes analytics, care management, patient engagement, quality reporting and an enterprise data warehouse. Its strengths are providing insightful financial and clinical information for the administrative staff of an organization.
Corporate headquarters: Dallas, Texas
Medecision gained wealth of experience in population health management by working with health plans and payers, and the majority of Medecision’s customers are payers using its products for care management. Provider customers range in size from midsize practices to large health systems.
Corporate headquarters: Salt Lake City
An Aetna company, Medicity offers a suite of acquired software and professional services that are geared to help provider organizations move from fee-for-service to value-based care.
19. NextGen Healthcare
Corporate headquarters: Horsham Township, Pa.
The company offers a vendor-agnostic PHM product with several applications geared toward preparing and analyzing data to make it useful for care coordinators and physicians. The platform was developed by EagleDream Health, which NextGen acquired.
Corporate headquarters: Eden Prairie, Minn.
Owned by UnitedHealth Group, Optum offers various population health management products and services, including care coordination and data analytics functionality. Optum customers tend to be large health systems supporting very large populations in risk arrangements that range up to full capitation.
21. Philips Wellcentive
Corporate headquarters: Alpharetta, Ga.
The Wellcentive brand is a longstanding player in the PHM market, offering a variety of functionality important to managing populations of patients. Customers range from midsize ambulatory practices to hospitals, ACOs, integrated delivery networks and large health systems.
