While a valuable part of chronic care management programs, traditional disease management and/or case management alone is reactive, episodic and not always clinically actionable. As a result, it often misses that moment when coaching and interventions are most effective.
During this web seminar, pioneering leaders involved with the Iowa Medicaid Congestive Heart Failure Population Disease Management demonstration share the breakthrough results they experienced by adding the Pharos Innovations model of remote patient monitoring to Iowa Medicaids care coordination program. Using industry best practice-level methodology, Iowa Medicaid reported industry-leading results, including:
- $13,000 per member savings from reduced healthcare service utilization for a total of nearly $3 million savings for the participant group, compared with a $2 million increase for the matched cohort group
- 24% reduction in hospital admissions, compared to a 22% increase for the matched cohort group
- 22% decrease in total bed days, compared to a 33% increase for the matched cohort group
During the live event, attendees will hear an in-depth explanation of how these results were achieved, as well as discussions about the most effective methods to measure results and the best ways to introduce care management programs and technologies at your organization.
Speaking Panel:
- William Appelgate, Ph.D. Executive Director, Iowa Chronic Care Consortium
- Randy Williams, M.D., Founder and CEO, Pharos Innovations
- Other speakers to be announced
