Chronic diseases have been called a health epidemic, and few would argue the country faces anything short of that.

Health I.T. is being brought to bear on the problem in a big way as private payers and the federal government align financial incentives and penalties with better chronic disease and population health management. But providers and payers have been beset with chronic I.T. problems, namely that data needed to monitor and intervene effectively in disease management hasn’t been collected, analyzed and acted upon efficiently. But there is hope.
