Population health management vendor ZeOmega has acquired HealthUnity, which operates a health information exchange that also offers master person index, referral management, and patient consent management software.

ZeOmega’s Jiva population health management platform delivers analytics, workflow, evidence-based content, and patient engagement capabilities required for organizations seeking to transition to value-based care.

"Combining the companies’ capabilities will create a powerful, rapidly deployable PHM infrastructure that payers and providers alike can use to drive value-based care, not only within their organization, but also within their communities," ZeOmega executives said in announcing the deal.

The goal is to match population health management with HIE to ease interoperability issues that hamper coordinated care. HealthUnity integrates with more than 35 electronic health record systems.
