A new company, Visions@Work LLC, has introduced software to automate the patient referral process.

Two veterans of NCR Corp.'s health care division, which includes the Galvanon patient self-service kiosks sold to provider organizations, lead Clermont, Fla.-based Visions@Work.

Pranam Ben serves as CEO and previously was a director of professional services at NCR. Diana Croft serves as president and previously was an enterprise program manager at NCR.

The new software, called Preferr, enables providers to initiate, receive and manage referrals electronically. The application supports the real-time, secure exchange of demographic and clinical information. It tracks referrals to identify and forecast patterns to better manage care and staffing, according to the vendor.

The remotely hosted software is available via a monthly subscription. More information is available at visionsatwork.org.

A major success factor for accountable care organizations will be linking caregivers across the spectrum of care delivery. If history is any indication, that's going to be an industrywide struggle.
