Project of the Year, Population Health: Village Family Practice, Houston

Analytics used in efforts to review charts, EHRs to improve patient care.

In addition to honoring Village Family Practice’s president, Clive Fields, MD, with a visionary award, the 40-clinican medical group is being recognized for achieving a more robust, evidence-based approach for reviewing charts and electronic health records.

The facility switched from manual reviews of charts to an analytics database that can identify miscoded or uncoded patient diagnoses, as well as mine and analyze patient records for missed conditions. Village Family Practice recently integrated pharmacy data into its analytics program to assist with refills and dosage.

Nationally recognized by the Centers for Medicare and Medicaid Services and the National Committee for Quality Assurance, Village Family Practice has cut Medicare admissions and costs so that they now are 20 percent to 45 percent less than costs of comparable practices in the area, and it’s reduced commercial costs by 15 percent to 20 percent.
