Web portal and analytics vendor MEDSEEK has acquired population health management firm SymphonyCare for an undisclosed price.

Birmingham, Ala.-based MEDSEEK sells physician, patient and employee Web portals, as well as well as health systems marketing and patient engagement software. It also owns data analytics vendor Third Wave Research Group which has technology to enable providers, payers, accountable care organizations and other entities to identify consumers and patients likely to need health care intervention.

Now, MEDSEEK is closing the loop with Madison, Wis.-based SymphonyCare’s population segmentation, risk stratification, care management and quality reporting software products. Combing the various product lines will enable the company to target and reach out to engage patients via print, the Web, social media, email and other avenues; enable care managers to build care plans, and give patients portal tools to follow the plans.
