Patient Education Vendor Launches Care Transition Program

Emmi Solutions LLC, a vendor of patient education and risk management software, has introduced a program to educate patients and track their health status as they transition to different care settings following hospital discharge.

Chicago-based Emmi offers Web-based interactive presentations on more than 130 medical conditions and related procedures. The presentations educate patients while providing informed consent documentation. Physicians using Emmi's content give their patients access codes to view presentations.

The new program, EmmiTransition, uses automated, interactive phone calls to keep track of patients’ health status following discharge. Patients are given health tips or urged to conduct daily activities such as checking their weight, and they also are asked specific questions to assess if any health problems are developing.

Four hospitals piloted the transition program with more than 100 congestive heart failure patients who received 45 days of communication. The program identified red flags that would have led to increased readmissions, says Louis Teichholz, M.D., medical director at Hackensack University Medical Center in New Jersey.
