NY Providers Using HIE to Coordinate Medicaid Care

Six provider organizations in western New York are using the Buffalo-based HEALTHeLINK health information exchange to better coordinate care for Medicaid patients.


Six provider organizations in western New York are using the Buffalo-based HEALTHeLINK health information exchange to better coordinate care for Medicaid patients.

The organizations already operate as “Health Homes” (medical homes) for patients covered under Medicaid. They include Greater Buffalo United Accountable Healthcare Network, Health Home Partners of Western New York, Health Homes of Upstate New York Western, Health Homes of Upstate New York Finger Lakes, Health Homes of Upstate New York Southern Tier, and Niagara Falls Memorial Medical Center Health Home.

Currently, 15 organizations within these health homes and 125 provider and staff users are participating in sharing information via HEALTHeLINK. Patients sign a consent form authorizing a care manager to coordinate their treatment and to access information from the HIE via a secure messaging service.

Insurers increasingly are encouraging medical homes, to improve care while lowering costs. Blue Cross Blue Shield of Michigan earlier this year estimated it saved $155 million during the first three years of its medical home program.
