Medicare Expands Service Areas for Telemedicine

The American Telemedicine Association is applauding a decision from the Centers for Medicare and Medicaid Services to expand rural coverage of telemedicine services, which CMS calls telehealth, reimbursable by Medicare.

This is good news and real progress. Kudos to those who worked to make this happen.

Though grateful and this reimbursement is a big step in the right direction, my criticism is that if you focus on the patient, the distinction between rural and metro are insignificant. Is it any less difficult for a person to travel to a follow-up or routine chronic care appointment in one setting versus another? It is a major and costly undertaking to make that doctor visit.

The criteria for telemedicine reimbursement should focus on outcomes (like compliance with care plans), and avoidance of hospitalization or re-admission versus physical distance. After all, the world is becoming digital

Still, we are making progress.
