Health payer software vendor Medecision has acquired Cerecons, an Anaheim Hills, Calif.-based vendor of Web-based care coordination, population management, and quality and outcomes reporting software for accountable care organizations and other risk-bearing entities. Terms were not disclosed.

The acquisition brings additional capabilities to Medecision as payers and providers increasingly move to new care delivery and reimbursement methods. Cerecons’ suite of software also includes clinical analytics and alerts, and patient portal software. The company has 72 risk-bearing payer and provider entities as clients, representing more than 60,000 primary care and specialist physicians and 1.5 million consumers. Philip Paul, CEO at Cerecons, now is an executive vice president at Medecision.

Medecision recently integrated its Aerial care management software to the claims clearinghouse of Availity to enable providers and insurers to conduct real-time transmissions of admission and discharge information. This will enable health plan and provider care managers to immediately initiate post-discharge care plans, according to the vendors.
