Medecision has linked its Aerial care management software to Availity’s network of providers and insurers to enable real-time transmission of admission and discharge information. This will enable health plan and provider care managers to immediately initiate post-discharge care plans, according to the vendors.
The vendors now are pilot testing the service with multiple payers and hospitals.
