The State of Indiana now permits parents and patients to access immunization records from the state immunization registry.

The state has rolled out the MyVaxIndiana Web site following a pilot program that included health departments, hospitals, physician practices and a public school. A vaccine record, authorized after an individual gets a personal identification number from their physician or the state health department, can be printed or faxed to the individual, downloaded as a PDF file, or downloaded into a personal health record.

Printed, PDF and fax copies are official state immunization records, according to the Web site. “The HL7 and PHR files are for personal use only and should not be used as an official document from the State of Indiana.”
