Guide Walks Practices through Clinical Summary Processes

Quality improvement organization Qualis Health in Seattle and the Washington & Idaho HIT Regional Extension Center have released a technical guide on providing patients clinical summaries of care after each office visit.

The guide explains 11 categories of content for the summary: identification, vital signs, encounter diagnoses, encounter orders, results of any tests available during the visit, updated medication list, current allergy list, problem list, chronic condition monitoring, and health maintenance information such as preventive care that is due.

The guide then walks through five major workflow components for the practice: preparing the care team, a pre-visit summary, synchronizing the patient and care team, productive interactions, and publishing the visit summary. The technical guide is available here.
