Organizations participating in the funding round include Caris Ltd., LAH Investments LP and Polaris Venture Partners. The vendor will use the proceeds, in part, to fund product development.
Phytel's software pulls data from various information systems to create a registry. The application then applies evidence-based disease management and preventive care protocols against the registry to identify patients due for care, and automates the notification process. The software also enables the tracking of patient responses and monitoring of compliance.
