Oklahoma Breach Hits 84,000

Saint Francis Health System in Tulsa, Okla., is notifying 84,000 patients and some employees of a breach of their privacy following the theft of an old computer at a former hospital in Broken Arrow.

An imaging center now operates out of the old hospital location. The stolen computer, last used in May 2004, was in the information systems room. The affected patients were treated at the hospital prior to 2004 and the stolen computer also held some employee records. Patient information on the password-protected computer included name, date of birth, Social Security number, mailing address, insurance data and diagnosis codes. The theft was discovered on Jan. 14.

The delivery system is offering to pay for a 1-year subscription to credit protection services from Equifax. More information and sample letters of notification are available here.
