Vendors Combining Capabilities to Support Population Management

Population health management software vendor Phytel and data analytics firm Verisk Health will integrate their products to develop a risk assessment/medical cost management system that uses claims and clinical data.

Phytel brings its Atmosphere Population Management platform that aggregates data from electronic health records and physician practice management systems help manage patient outreach, identify gaps in care, stratify risk, provide care management and manage care transitions, and evaluate performance. The Provider Intelligence system from Verisk Health identifies and targets inappropriate use of resources, in-network and out-of-network costs and utilization, and physician and practice strategies that improve care and maintain business stability.

Combined, the technologies can help determine risk characteristics of a patient population and targeted interventions, identify individual patients at greatest risk and non-value-added treatments to avoid, and assess a practice’s effectiveness in population management and how it compares with peers, according to the vendors.
