The theft occurred during the weekend of Oct. 22-23 and the practice notified patients on Dec. 5, The San Antonio Express-News reported in late December. Information on the computers included names, Social Security numbers, birth dates, disability codes and diagnoses.
The letter to patients encouraged them to place a fraud alert on their credit reports and monitor the reports. The newspaper could not determine if affected patients are being offered paid credit/fraud protection services. The practice referred Health Data Management to local attorney Celeste Lira, who was not immediately available for comment.
