Physician and hospital software vendor Allscripts has introduced new software to support the enhanced care coordination across provider settings required under accountable care organizations and other value-based treatment/payment programs.
The Care Director product is an extension of the vendor’s Care Management suite of software, enabling “true care coordination across all settings,” according to an explanation. Patient data can be imported directly into Care Director, or pulled from Allscripts’ Sunrise Clinical Manager product and any third-party EHRs that support HL7 standards.
The new product supports identifying gaps in care and at-risk patients, transitions of care, and data analytics to measure care process improvements and patient outcomes, according to the vendor.
