The State of New York has awarded nine grants totaling $59.6 million to organizations that will implement information technologies to support the medical home model of care.

Under the model, primary care practices serve as the focal point for patient care coordination. The model advocates extensive use of I.T. to document and coordinate care across all providers and settings.

The new grants are funded under the state’s Health Care Efficiency and Affordability Law of New York State, called HEAL NY. Through the law, the state has provided tens of millions of dollars in grants in recent years for health I.T. initiatives, particularly health information exchanges.

The nine new I.T. grants, which will assist in implementing electronic health records and connectivity initiatives, were awarded to hospitals, health information exchanges and other collaboratives, peer review organizations, and public health agencies across the state. The grants range from $5.3 million to $7.0 million.

The I.T. awards were among a total of $436 million in new HEAL NY grants for a variety of programs to improve the state’s health care system. More information is available at http://ny.gov/governor/press/press_0925091.html.
