The Brooklyn Health Information Exchange will offer participants software and disease management services from ActiveHealth Management, a New York-based subsidiary of Aetna Inc.

The HIE is deploying the vendor’s clinical decision support and personal health records software. The PHR will be able to notify consumers of alerts and wellness reminders and solicit information on allergies, over-the-counter medication use and other information to identify ways to improve care. ActiveHealth and the exchange also will collaborate on a pilot program to develop customized care plans for geriatric patients and those with HIV/AIDS.

The Brooklyn HIE includes Maimonides Medical Center, Kingsbrook Jewish Medical Center and several nursing homes and home health agencies. It has received two state grants totaling $12.7 million.

Other vendor partners include Mason, Ohio-based MedPlus Inc. for a clinical Web portal, information exchange platform and document imaging and management software; and Chicago-based Initiate Systems Inc. for master patient index software. More information is available at bhix.org and activehealth.net.
