Mountain View, Calif.-based Acesis Inc. has released an electronic health records system designed to help providers better capture patient data.

The start-up vendor's application, Point-Of-Care, enables the creation of encounter notes via a series of documents that automatically are altered based on prior responses. Further, the system uses standardized medical vocabularies to help clinicians standardize the reporting of data.

The Web-based system is available via the application service provider computing model. It also can be used on mobile hardware.
