In 15 months from January 2014 into March 2015, the healthcare industry had 15 separate major breaches of protected health information that affected well over 100,000 individuals. At least nine of the breaches were caused by hacking and it appears a couple others were, but wording on the HHS Office for Civil Rights’ breach notification web site is unclear (e.g., “theft of network server”).
