Clinically integrated networks face many challenges to simultaneously focus on the individual patient and the population in order to provide high-quality, cost-effective patient care. In order to achieve high-value care, clinically integrated networks must synthesize and analyze data to address payer and ACO metrics, manage cost through utilization, and transparently demonstrate quality. 

The presentation will define how an organization can condense disparate data sources to create timely, accurate, and actionable information allowing the healthcare team to address individual patient needs and improve the population's health at-large. Attendees will be walked through an explanation of the integrated model as well as case studies on how patients are uniquely identified through multivariate risk analysis and how the integrated network can effectively manage the patient and population through analytics to limit the risk of over-utilization and demonstrate high-quality care.
